Liability waiver This waiver applies to every participant in your order.
In consideration of being permitted to participate in any event hosted by Whole Hearts in Motion (the "Event"), I, the undersigned, acknowledge that participation in a 5K run/walk involves inherent physical risks, including but not limited to injury, illness, or in rare cases, death. I certify that I (and any minor participants I am registering) am physically able to participate and have consulted a physician if necessary.
I voluntarily assume all risks associated with participation, on my own behalf and on behalf of any minor children I am registering. I hereby release, waive, and discharge Whole Hearts in Motion, its founders, volunteers, sponsors, and affiliates from any and all claims, liabilities, or causes of action arising from participation, including those caused by ordinary negligence.
I grant permission for photographs or video taken at the Event to be used by Whole Hearts in Motion for promotional and educational purposes. By typing my name below, I acknowledge that I have read this waiver, understand it, and agree to be bound by it on behalf of every participant listed above, and that this constitutes my electronic signature.
